A missed revenue-cycle target is a starting point for an investigation. Before choosing a fix, confirm exactly what the number measures and whether the supporting data is complete.
1. Write the measurement definition
Name the unit, population, date field, exclusions and source. First claim submissions, first adjudications, appeal decisions and cash receipts represent different events. An encounter, claim line and payment group are different units.
2. Validate the inputs
Reconcile counts and dollars to independent source totals. Remove duplicate events using a documented key, preserve reversals and retain unresolved inventory. A missing extract is missing data, not a zero population.
3. Inspect both sides of the calculation
A ratio can improve because its denominator grows. A completed-item average can look better while old open items remain untouched. Review numerator, denominator, absolute dollars and unresolved inventory together.
4. Trace a specific cause
For authorization readiness, inspect provider, service code, valid dates and remaining units. For denial performance, separate payer processing errors from confirmed upstream defects. For cash applications, reconcile posting, bank settlement and ledger records independently.
A synthetic example
180 valid authorizations among 200 services requiring authorization gives 90.0% readiness. After a workflow correction, 190 of 200 gives 95.0%. That is a gain of five percentage points. It still falls short of an illustrative 98.0% local target. Check service delays and related denials before calling the correction successful. These teaching values are not universal benchmarks.
5. Assign the correction and verify it
Record the confirmed cause, supporting evidence, specific action, owner, due date and follow-up window. Recalculate with the same definition and inspect a balancing measure. A new status label or a favorable appeal decision does not prove recovered cash.
